The general idea for link exchange is to find sites that are related in content with your site and to place his link on your index page so that your link will be posted on his site. That's something like advertising. The result is that search engines, will place on top of the search list the domains for which it finds the more reciprocals to its url.
First you have to look on searching engines (google, yahoo, msn) SEO ELITE and other sites (you can also take a look at other sites resources), and find the best sites to exchange links with and this means to check if the links page has a link from the homepage, if the URL's name is a good one and the webmaster is not a cheater. Also, it's very important to find sites related to your web site that DO NOT have a large directory of links posted. You have to link to sites that have less than 100 links on their link page. It is said that a page rank equal or higher than the page rank of the site you are working on is good for the link exchange procedure. But there are good sites just born which have no PR but which will grow. Anyhow an index back from a site is good no matter the PR of the specific site.
Another thing you have to do is verifying if your partners still have a link back to you; if they don't, you have to remove their link from your link pages, and send them a declining email.
You also have to check if reciprocals are good and you do that every 2 or 3 weeks. If link back is wrong you have to search again on the site to see if maybe reciprocals are moved to another address from that link. Reciprocal listings can be verified either manually in a "View All Links" tool, or using SEO tools which can spider the websites of all your link partners searching for your link and after that it tells you if the link to your website is still active; the URL of the page where your link was found; the PR of that page; the title of that page; number of outbound links on that page; the anchor text of your link.
